Woman dies in head-on crash on Hwy 191

A woman was killed on May 10 in a three-car crash on Highway 191 north of Monticello.

Zuhra Samadi, age 26 of Salt Lake City, was killed while attempting to pass – in a no-passing lane – a Ford F-350 that was pulling a 25-foot enclosed trailer.

Her vehicle, a Chevy Tahoe, collided head-on with a Dodge Ram pickup traveling in the opposite direction.

The Tahoe was then pushed into the Ford’s trailer where both vehicles and trailer continued northbound veering off to the shoulder. 

Samadi died on scene as a result of her injuries, while a two-year-old passenger was taken to a local hospital with non-life threatening injuries.

The driver and passenger of the Dodge Ram were also taken to a local hospital with minor injuries.

All five occupants of the Ford were treated and released on scene.

Crews were on scene for nearly four hours through the night. Notification to family members of the deceased is still being made.
